To complete this form and our paperwork process, you will need the following items scanned into computer files and ready to upload into our system.
For new staff, please upload all items as required. For returning staff, we may already have some of these items on file for you. However, items marked with an ** must be completed and submitted every year. For items not marked with an **, only submit if you need to update your information since your last summer of employment (ex: a passport renewal, new certifications earned, etc.).
** 1) Your signed employment contract
** 2) Completed and signed Form W-4 (U.S. citizens and residents only)
** 3) GXG Staff Medical Form
** 4) GXG Counseling Questionnaire (only if you have been under the care of a psychiatrist or psychologist in the past 12 months)
5) Completed Section 1 of Form I-9 (U.S. citizens, permanent residents, and aliens authorized to work in the U.S.)
6) Your passport data page(s)
7) First Aid, CPR, WFR, EMT, etc. certifications that will be valid during your contract period
8) 1-3 high quality, wide-angle photos of yourself wearing professional attire with no hats or shades and a clear shot of your face (see the GoBeyond Staff page for examples) and a short staff bio
